Liability Waiver and Release
I, the undersigned parent or legal guardian of the above-named minor child (“Participant”), hereby give permission for my child to participate in all activities offered by ReadySet Educational Consulting, LLC, which may include both indoor and outdoor activities at The Pavilion at Central Park.
I understand and acknowledge that participation in workshop activities may involve risks including, but not limited to: falls, injuries, physical exertion, exposure to weather conditions, and contact with plants or insects. I acknowledge that these risks are inherent in recreational activities and I voluntarily accept and assume them on behalf of my child.
I agree to release, waive, and hold harmless ReadySet Educational Consulting, LLC, its directors, staff, volunteers, representatives, and agents, as well as the hosting park and local municipality, from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, injury, or death that may occur to my child while participating in workshop activities.
Medical Treatment Consent
In the event of injury or medical emergency, I authorize the workshop staff to seek medical treatment for my child, including calling emergency services. I understand that I am responsible for any medical expenses incurred.
Acknowledgment of Program Status
I understand and acknowledge that ReadySet Educational Consulting, LLC and Ready Set Middle School readiness workshops are not licensed childcare providers in the state of Colorado and are not subject to childcare licensing regulations as defined by the Colorado Department of Early Childhood. This workshop is a short-term, educational readiness program designed to help students transition into middle school. It is not intended to provide childcare or long-term supervision services.
